We will engage with subject matter experts to understand data requirements and applications that support health centers in advancing population health management and value-based care.

Session Objectives:

  • Gather input from health center participants on data needs for Population Health management and Value-Based Care

  • Define data and analytic application features and functions needed to support population health management and value-based care

  • Identify gaps between currently available application features and functions and health center needs to support population health management and value-based care
