The goal of this four-part series is to provide health centers with a review of vetted data applications used for population health management and value-based care needs currently available in the marketplace. We will engage with subject matter experts to understand data requirements and applications that support health centers in advancing population health management and value-based care.

Session 2 Objectives:

• Review Population Health Management and Value-Based Care Applications requirements list

• Receive an initial reaction and feedback from attendees on the requirements list

• Determine with attendees a recommended process to capture detailed feedback from CHCs to finetune the requirements list
